Signing Up

To sign up, please fill out the following required forms:
Participant Application.  To be completed by all riders in the group (including the driver and bookkeeper).  You must have a minimum of 7 participants including driver.

Selection Criteria for the Driver and Bookkeeper.

Driver's Application.  To be completed by all drivers (including the back-up drivers).

Bookkeeper Application.  To be completed by the bookkeeper of the van group (can be a driver or rider).

Vanpool Database Information.  You will need to provide specific information about the miles you will be traveling.

Participant Change Form. To be completed by riders when they have a change in their status. 

The completed forms can either be emailed as attachments to wkarsch@rideuta.com or faxed to: Attention: Wendy Karsch, 801-287-5031
